The Statewide Parent Advocacy Network (SPAN) seeks proposals from teams that include community-based organizations, health and/or mental health professionals/service providers, and family organizations/family leaders interesting in collaborating to improve outcomes for their community’s children and youth with special health care needs (CYSHCN). Proposals must address one or more of the six core outcomes for CYSHCN: families are engaged at all levels and satisfied with services; early and continuous screening to identify special healthcare needs; coordinated care within a “medical home;” community-based services organized for easy access; adequate public and private insurance; and/or effective transition to adult systems of care. Proposals must demonstrate how the applicants will work to improve outcomes, particularly within diverse and underserved communities, and how the project will include families of CYSHCN in all aspects of planning, implementation, and evaluation. Diverse and traditionally underserved groups are encouraged to apply.
Maximum Award: $2,500.00
Who can Apply: New Jersey non-profit and for-profit community and professional groups or family-led groups that are partnering with a minimum of one primary care pediatric/family care provider and two community partners within one community and family organization(s)/ family leaders focused on improving outcomes for Children and Youth with Special Health Care Needs (CYSHCN).
Project Period: 12 months

Application and Submission Procedure

Element 1 – Capacity Summary (15 points)
All Applicants must submit a capacity summary. Provide a brief description of your organization, including organizational history, mission statement, a description of the programs you offer, the geographic area and population you serve, service site and how your organization is funded and your area(s) of interest. Applicants must describe their governing board and/or officers.
Element 2 – Diverse community and family engagement in planning (15 points)
Describe how the applicant and the applicant’s partners will work to improve outcomes for CYSHCN and include family participation and strategies to address issues of cultural competency within their proposed projects.
Element 3 – Project Description (35 points)
Describe the project for which you are seeking funding. Who and how many will be served? What and who will provide the service? What current organizations/groups have your partnered with or what organizations/groups do you plan to partner with? How will known disparities be addressed when prioritizing and implementing strategies? What resources will be used to execute and meet the requirements of the grant award?
Element 4 – Budget (15 points)
Include a detailed budget indicating all income, expenditures and the source of income/funding.
Element 5 - Evaluation (15 points)
Include a description of the type of criteria your organization will use to evaluate the success of the project, using measurable goals and objectives, evaluation tools, etc.
Element 6 – Sustainability (5 points)
Describe how you will sustain efforts after the mini-grant ends.
